Lesions on MRI in Patiens of Head Injury Without Mass Image on CT Scan


Abstract
Thirty five patients of closed head injury were studied by magnetic resonance imaging(MRI) during long term follow up of a head injury. Abnormality indicating primary brain damage are found in 31 patients compared to 27 patients with computed
tomography. Cortical contusion is the most common finding, irrespective of the effect of injury on the level of consciousness. Intraaxial lesion were found in 2 patients which had lost consciousness briefly and arrived at hospital in alert state
with
negative CT finding. Lesions in deep white matter of the hemisphere and deep gray or brain stem were highly correlated with duration of impaired consciousness and patient's final outcome. We also found lesion in stem without lesion in deep white
matter
which Showed different clinical course from cases of stem lesion combined with deep white matter lesion. These findings suggested strong possibility that primary brain stem injury and lesions in cerebral hemisphere may be primary factor in
impaired
consciousness of head injury.
